Editors note: As of 17th May, this form is no longer in use. Red/Amber/Green Lists are now in use – see https://economyclassandbeyond.boardingarea.com/2021/05/07/breaking-government-reveals-its-green-list/
This page will remain up for historical purposes.
For those who are intent on departing the United Kingdom, as of today, you will be required to download and fill in a form, detailing your reasons to leave to the United Kingdom.
The forms are live at https://www.gov.uk/guidance/coronavirus-covid-19-declaration-form-for-international-travel. You will need to complete this form to declare the reason that you need to travel abroad.
You don’t need to complete the form for travel within the UK, to Ireland, the Channel Islands or the Isle of Man.
Once completed, you will need to either print a copy of the form or save it to a mobile phone or accessible device. The form may be inspected before you depart, and you might need to bring additional evidence for your reason for travel.
In terms of offences, entering a port of departure to travel internationally without a completed form is a criminal offence, for which you could be fined. If you try to travel abroad without a legally permitted reason, you may also be fined for breaching the stay at home requirements.
There are jobs that do NOT require this form to be completed. These are listed at https://www.gov.uk/government/publications/coronavirus-covid-19-travellers-exempt-from-uk-border-rules/coronavirus-covid-19-travellers-exempt-from-uk-border-rules.

I can’t see this form on the link – do we still need to fill it in?

It seems the requirement for the form has gone.
I’ve updated the header of the article so people see they don’t need to fill one in.
